1. First and last name
2. Work site
3. Current first responder affiliation

Take a picture and submit one of the following:

• ID Card with valid date
• Pay stub from within last 90 days
• An official letter of service

Note: if the organization shown in a paystub doesn't match your work site name, you may also be asked to upload proof that the paystub organization and work site are connected.

You need to be currently working as an eligible first responder.

Sometimes the brands we work with will decide to limit the offer to only some types of people in the first responder community. If on the form you don't see an option that is a close fit, then that means you unfortunately are not eligible for this offer.
